About the role/Job Description
- As a Support Coordinator team leader you will;
- Provide Specialist support coordination and Support coordination to NDIS participants;
- Have highly developed collaboration skills and experience managing complex needs with a network of services;
- have well-developed information management skills with the ability to work towards innovative solutions to solve problems;
- Ability to provide leadership in the form of coaching, guidance, and support to develop a multidisciplinary team.
- Oversee all aspects, of co-ordination and case management.
- Champion organisational KPI's to deliver outcomes within specified timeframes and to a high standard;
Essential & Desirable Criteria
The ideal applicant will have;
- Ahpra Registered, or Degree qualified in Registered Nurse, Social Work, Psychology, Physiotherapist or similar relevant discipline
- Minimum of three years’ experience in a similar role;
- Well-developed understanding of the NDIS and issues affecting people with a disability;
- Demonstrated experience in collaboration and partnership development to achieve positive outcomes;
- Able to work independently and within a team environment;
- Advanced risk management skills and experience managing behaviours of concern;
- Advanced computer literacy and written/verbal communication skills
- Ability to maintain high standards of safe clinical care;
- Professionalism and alignment with the values and ethos of Newcastle and Hunter Community Health;
- Ability to work independently and effectively manage time;
- Australian unrestricted Driver's Licence;
- Criminal record check (or willingness to obtain same);
- Working with children check (or willingness to obtain);
- NDIS worker screening Check;
- 2 Contactable Manager referees;
- Excellent communication skills;
- Covid-19 Vaccination to Department of Health requirements;
