The Role
The role of the Support Coordinator is centred around supporting clients to understand and implement the funded supports in their NDIS plans, as well as linking them to appropriate services. You will work in collaboration with clients and their families/representatives to build up their capacity to manage the support within their NDIS plan. In doing so, you are tasked with going above and beyond to support your clients in reaching their goals and pursuing their dreams.
You will be responsible for:
- Assist NDIS participants to implement NDIS Plans and coordinate their services and supports.
- Complete all necessary administration to the required standard, ensuring full compliance.
- Identify, develop, coordinate, and facilitate access to supports and services alongside their allocated NDIS Clients. This includes the coordination of assessments and reports as per the NDIS guidelines.
- Assist the participants and their families in understanding and reviewing their NDIS Plans to ensure they are getting the maximum benefit and personalised care
- Build the capacity of the participants to achieve greater independence to self-direct services and supports in the longer term
To be successful in this role you will have:
- Qualifications in the field of Disability or Social Service sector
- Demonstrated experience providing support coordination and case management with knowledge and understating of the NDIS framework
- An understanding of the impact of disability and an ability to access specialist skills and resources to support clients with complex support needs
- Ability to interpret an NDIA plan and budget, and recommend service options within the guidelines of the plan and budget
- Excellent oral and written communication skills
- Highly organised and detail-oriented
- Required to work well both independently and cooperatively as a member of a team, the position requires a person who displays self-discipline with the ability to avoid distractions and diversions in order to manage their own time effectively.
Key responsibilities:
- Work with clients, families, and carers to identify and access opportunities for skills development, education, and employment
- Support and encourage clients to achieve their goals and provide support coordination services in line with their NDIS plans
- Develop networks and collaborate with stakeholders to proactively identify, plan, implement and evaluate supports.
- Prepare and maintain up-to-date and accurate case management notes and reports using clear and concise language on client progress.
- Liaise with a wide range of stakeholders, community services, government agencies, customers, and service providers.
- Attend and contribute to team meetings, discussions, and planning. Generate ideas and options, whilst maintaining flexibility and receptiveness to change and alternatives.
