About the role:
As a Support Coordinator, you will be responsible for helping NDIS participants achieve their personal goals through Coordination of Support. Your key functions will include:
- Collaborating with Participants to develop and refine their NDIS goals, explain how to use their NDIS plans effectively to achieve their goals, and coach them in capacity building and increasing their independence
- Building strong and effective relationships with external providers.
- Monitoring of NDIS plan budgets and support services to ensure plan budget is utilised effectively and services are working towards the NDIS participant to achieve their plan goals.
- Liaising with participants and all relevant stakeholders
- Ensuring participants are receiving qualitative value for money and encourage self-direction.
- Monitoring plan outcomes and expenditure.
- Meeting set targets, KPI's and caseload in a timely and effective manner
- Staying updated on industry changes.
- Maintaining accurate and up to date case notes on Participants needs and progress
To be successful in this role you will need to:
Understand mainstream, community, formal, and provider support available to NDIS participants
- Communicate technical language used in NDIS plans effectively
- Be open to learning and show empathy
- Work independently in an agile environment, remaining flexible and adaptive
- Deliver people-centered services effectively
- Demonstrate organizational and problem-solving skills
- Possess excellent interpersonal, teamwork, and communication skills
- Have high computer literacy
- Have experience as a Support Coordinator (essential)
- Possess experience in supporting high-needs clients (desirable)
