NDIS Support Coordinator

Position Overview: We are seeking a dynamic and results-oriented NDIS Support Coordinator with a strong background in customer service to join our growing team. The ideal candidate will play a key role in connecting with NDIS participants, understanding their unique needs, and providing tailored support coordination services. Key Responsibilities:
- Engage with NDIS participants to assess their goals and support needs.
- Develop and implement individualized support plans in collaboration with participants.
- Actively promote and sell our support coordination services to potential clients.
- Utilize your sales and customer service skills to build and maintain strong relationships with NDIS participants.
- Actively obtain new client referrals through outreach efforts, networking, and strategic partnerships.
- Track and manage new client referrals, ensuring accurate and timely documentation.
- Achieve and exceed set monthly Key Performance Indicators (KPIs) related to new client acquisition and service delivery.
- Provide support to participants to increase their independence, social participation, and economic participation for better control of their lives.
- Assist clients, families, and carers to connect with services both mainstream and within disabilities.
- Develop, monitor, review, and report on your client’s NDIS plans and share progress with key stakeholders.
- Work within the funding allocated and regularly monitor and track billable coordination hours.
- Build and maintain relationships with our clients, their families, staff, peers, other services, and stakeholders.
- Work collaboratively with our team to ensure seamless service delivery.
- Stay informed about NDIS policies and updates to provide accurate information to participants.
Qualifications and Skills:
- Previous experience in the community sector and support coordination is preferential.
- Understanding of the NDIS framework and support coordination.
- Proven ability to actively obtain and track new client referrals.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong organizational and time management skills.
- Passion for making a positive impact in the lives of NDIS participants.
Compensation:
- Base Salary: Competitive base salary commensurate with experience.
- Additional Performance-Based Bonuses.
