Focus ACT is a provider of disability supports, out-of-home care, Support Coordination, Community Housing and Allied Health in Canberra, ACT, and NSW surrounds.
We have provided in-home support services for 30 years. We believe in witnessing the people that we support to live their life, making them visible in their community, and always finding a better way.
Focus is growing its Support Coordination team and is looking for the right person to join our team.
About the role
Focus' Support Coordinators assist people to access the services they need to achieve their goals.
Facilitating choice and control for people, co-designing plans to success, creating and nurturing connections with effective service providers are all day-to-day activities of this role. The Support Coordinator is an exceptional communicator who has a competent understanding of the clients' informal support networks made up of friends, acquaintances, family, guardians, and important relationships.
As a confident ambassador of Focus, this role will work to secure referrals and assist the business development activity.

About you:
You are excited and passionate about a modern disability sector, with a drive to take your career to the next level. Ideally, you are working in the sector already with a working knowledge of supporting people with disability.
You will also:
- Have a genuine passion for to assist people with disability to live their life;
- Be diligent to perform with responsiveness and active engagement;
- Be experienced in person centred planning
- Execute effective communication;
- Be energetic, self-motivated and have a flexible and adaptable work style;
- Have well-developed organisational and time management skills, including the ability to prioritise workload;
- Develop and maintain highly productive work relationships with clients, guardians and colleagues;
- Working knowledge of Microsoft Office Suite.

Essential Criteria:
- Demonstrates ability to work individually with only general supervision and/or in a team environment

- Proficient in the use of Microsoft Office Suite and general communication technology

- Ability to obtain and hold a WWVP
- Must be willing to undertake a police check
- Fully comprehensive insured vehicle
