Summary:
We are seeking a highly motivated and experienced NDIS Support Coordinator to join our team. The successful candidate will be responsible for providing support and guidance to individuals with disabilities and their families, ensuring they receive the best possible care and services under the National Disability Insurance Scheme (NDIS)
The successful candidate must reach a set KPI each fortnight for billable hours to be maintained
The successful candidate will be allocated a company phone and laptop for work use
The role will be a hybrid mix of on-site and In-the-field work, as you will need to meet participants, stakeholders, etc
The position is casual but may lead to permanent for the right applicant
Responsibilities:
- Conducting assessments and developing individualized support plans for NDIS participants
- Coordinating and monitoring the delivery of services and support to NDIS participants
- Liaising with service providers, healthcare professionals, and other stakeholders to ensure the best outcomes for NDIS participants
- Providing information and advice to NDIS participants and their families on available services and supports
- Maintaining accurate records and documentation in accordance with NDIS guidelines
- Participating in team meetings and professional development activities to enhance knowledge and skills
Qualifications:
- Minimum Certificate IV or Higher in a relevant field such as disability, social work, psychology, Community Services, etc
- Demonstrated experience in the disability sector, particularly in NDIS support coordination
- Strong knowledge of the NDIS framework and guidelines
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong organizational and time management skills
- Current driver's license and access to a reliable vehicle
