ABOUT THE ROLE
We are currently seeking a NDIS Support Coordinator to join Carers ACT. This role provides direct service to participants with a disability and their carers by providing support coordination and pre-planning consultation. The successful applicant will be responsible for:
- Working closely with participants and their carers to ensure they understand their NDIS plans, including their goals, available funding, and the range of supports they can access.
- Coordinating various services and supports to create a comprehensive plan tailored to the participant’s needs, including connecting them with NDIS-approved providers, community resources, and other relevant services.
- Supporting participants and their carers to build essential skills and advocate for their best interests ensuring they are supported through unforeseen and changing circumstances.
- Marketing appropriate services to prospective clients
- Building and maintaining internal and external stakeholder relationships with potential and existing service providers.
- Maintaining accurate database records
- Actively encourage and support a seamless service for carers and recipients
ABOUT YOU
We're looking for a compassionate and empathetic individual to fill this role. The ideal candidate will have strong communication skills to engage and collaborate with participants, families and carer. They are detail orientated and be skilled in understanding the needs of their participants and services to support them. Your qualifications and skills include:
- Advanced understanding and experience in working with NDIS plans and services, with a minimum of 2 years' experience.
- Qualifications in relevant field is highly desirable, with a minimum of 2 years demonstrated experience in the community services sector.
- Ability to build and nurture relationships and networks with participants and their carers as well as disability service providers.
- Demonstrated computer literacy (MS Office)
- Highly developed communication and interpersonal skills
- Well-develop customer service skills
Additionally, you meet the following essential requirements:
- Current Working with Vulnerable People Registration (with NDIS activities if issued after July 2021) or willingness to obtain.
- Up-to-date COVID vaccinations
- Current Police check
- Current Driver’s Licence (essential to undertake client meetings at external locations).
