Job Summary:
We are seeking a dedicated and compassionate NDIS Support Coordinator to join our team in Kununurra, Western Australia. The successful candidate will be responsible for providing support coordination services to individuals with disabilities who are participants of the National Disability Insurance Scheme (NDIS). This role involves working closely with clients, their families, and other service providers to ensure that the needs and goals of each participant are met.

Responsibilities:
- Conduct initial assessments of participants' needs and goals
- Develop individualized support plans in collaboration with participants and their support networks
- Coordinate access to relevant services, supports, and resources as outlined in the NDIS plan
- Monitor progress towards goals, making adjustments as needed
- Provide advocacy on behalf of participants when necessary
- Maintain accurate records/documentation in line with NDIS requirements
Qualifications:
- Previous experience in working in a NDIS Support Coordination or disability support role.
- Demonstrated skills and ability in engaging and working with clients with a disability, family members and service providers to identify solutions.
- Experience liaising with external service providers, community-based service providers and organisations.
- Demonstrated experience comprehending and working within a budget.
- Ability to prioritise work, manage time and meet deadlines.
- Extensive experience in the use of the Microsoft Office suite and client management systems.
- Knowledge of, and experience working with remote Aboriginal community services within the Health and Social Services sector.
Preferred Qualifications:
Tertiary qualifications in Disability Services, Social Services, or related fields.
