By helping Participants navigate government and community systems, we and increase their social, economic and cultural participation, we support them to reach their full potential and live as independently as possible.

  • Psychosocial Recovery Coaching

Specialist Support Coordinator

Avenues Australia is looking for an entry level Specialist Support Coordinator. Through their sound knowledge and ability to solve complex service delivery problems and identify opportunities in a complex support environment, the Support Coordinator is able to navigate complexity, manage higher caseloads and make suitable recommendations to our NDIS clients to better meet their individual needs. Not only will you support our clients and the carers to manage the NDIS Plan, but you will also have a proactive and outcome-led approach to help them build capacity, chart a path to independence, acquire new skills, receive quality care and/or keep safe. You will have the ability to:

  • Build rapport with all stakeholders quickly while navigating complex issues and having difficult conversations.
  • Work collaboratively with participants, their families, carers and service providers to design, implement and adjust a support plan, including crisis planning.
  • Collaborate with the broader services system, in particular, mainstream services and ensure these work in unison with their NDIS Plans.
  • Advise more junior or inexperienced staff.
Responsibilities

The Specialist Support Coordinator is responsible for:

Support Connection and Coordination

  • Support clients and carers to understand their NDIS plan, goals and objectives.
  • Support clients in developing, implementing, and monitoring a support plan, including crisis management and the risks associated with the breakdown of support.
  • Working closely with clients and carers to identify their needs, personalise support and ensure they receive appropriate services.
  • Promoting and fostering independent decision-making, including choice and control.
  • Activity supporting and managing the sometimes-competing nature of dignity of risk and the duty of care of complex clients.
  • Coordinating and liaising with service providers, reviewing goals and jointly determining new ones with the client.
  • Coordinate the supports in an individual’s NDIS plan by:
  • - researching, coordinating and managing complex and intersecting supports to suit individual needs across multiple providers while ensuring client choice and control; and
  • - working with mainstream service providers, including negotiating services to ensure service obligations are met.
  • Coaching clients and/or carers to build on knowledge, skills, and decision-making with the aim of greater choice, control and independence in managing their personal choices.
  • Arrange any assessments needed to determine the nature and type of funding required.
  • Decide the budget for each support type and advise any relevant plan manager or service provider of the breakdown of funds.
  • Liaise with plan managers and/or NDIS to establish the appropriate claim categories and attribute funds accordingly.
  • Assist clients to prepare for plan reviews by helping them assess whether they achieved their goals and got value for money. Identify solutions to problems experienced in implementing their plan and to consider new goals.
  • Assist clients to communicate with the NDIA when required to support their plan requirements.
  • Resolve any crisis and breakdown of supports by linking participants with the appropriate services.
  • Actively assessment and manage all risks associated with supporting complex clients.

Operational

  • Demonstrate capacity to manage a high case load.
  • Demonstrated capacity to work efficiently and to complete administrative tasks.
  • Ensure the completion appropriate documentation as required.
  • Maintain good quality documentation of clients.
  • Monitor and report client outcomes to the NDIA through writing quality reports that are an accurate
  • reflection of the client’s complex support needs.
  • Actively participate in the development, implementation, monitoring, regular review and reporting
  • of client programs with providers including determining risk and preparing assessments, review client records and understand individual needs, review client goals and measure individual progress and document outcomes.
  • Promote Avenues Australia as a NDIS Support Coordination provider to participants and other stakeholders

Underpinning Knowledge

  • Knowledge of disability types and symptoms.
  • Sound understanding of choice and control, dignity of risk and duty of care.
  • Working knowledge of the service support providers including mainstream, community-based, and
  • specialist.
  • Willingness to understand the NDIS Pricing Arrangements and Price Limits Guide in detail.
  • Understanding of complexities around behaviour support management.

Other Duties

  • Conduct other duties as directed.
Qualifications
  • Social Work, Psychology, Occupational Therapy, any allied health degree or other equivalent post graduate qualification.
  • Current Western Australian Driver’s License and ability to undertake travel within the Perth Metro as required

Upon acceptance into the role, the candidate will be required to obtain the following

  • Current First Aid credential
  • Current Working with Children Check
  • NDIS Worker’s Screening
