Job description

Developmental Disability WA is seeking  a like-minded person  to join our team of  experienced Support Coordinators and Specialist Support Coordinators in a part time or fulltime role covering the northern suburbs of Perth.

You will be currently working as a Support Coordinator and importantly, you will have values that are aligned with ours. You will have an understanding of the Human Rights Convention of the Rights of People with Disability and a deeply held belief that all people have value and all have the right to be included (and supported as needed) in their local communities, schools and workplaces. 

You will be a fast learner and keen to attend training, induction and team meetings. You will also need to be savvy with technology (our systems are all cloud based), have attention to detail, good record keeping, good problem solving and research skills and be a strong communicator.

We are offering a competitive salary for this role and as a not for profit we also offer the benefits of salary sacrificing to our team up to save on tax up to $18,500 pa.

What you will need

  • A minimum of 12 months experience working as a Support Coordinator, ideally with experience in complex cases 
  • Tertiary qualifications are desirable - Allied Health (Occupational Therapy), Social Work, Counselling, Nursing or Human Services (and / or significant previous experience as an NDIS Support Coordinator)
  • Able to work as a Level 2 and ideally Level 3 Support Coordinator.
  • Experience working with the community sector and a strong understanding of NDIS operational guidelines.
  • Excellent communication, time management, and organisational skills. 
  • An ability to assess and respond to individual needs.
  • The ability to communicate effectively with a diverse range of people to build trust and develop rapport.
  • Familiarity with local services, organisations and other opportunities within the general community.
  • Ability to use Microsoft Office programs
  • Completion of the NDIS Worker Orientation Module
  • Satisfactory Working with Children's Check (Employee).
  • NDIS Worker Screener Check Certificate.
  • Current Police Check (last 2 months).
  • Current Driver's License and reliable vehicle (kilometrage costs are reimbursed).
  • A family role or having a lived experience of disability will be an advantage.

Tasks & responsibilities

  • Assist NDIS participants to coordinate supports and services in line with their NDIS goals. 
  • Work with NDIS participants, families and other stakeholders to implement, monitor and review NDIS plans.
  • Working to build capacity in participants, their informal supports and carers, so they may experience increased independence, opportunity, choice and control.
  • Coordinating person-centred support through liaising with providers and support services in line with a client’s NDIS goals.
  • Adhering to NDIS legislation, policy and procedure, at all times.
  • Supporting participants to build capacity so they may experience a greater degree of choice and control in their lives.
  • Writing comprehensive review reports and supporting participants through the NDIS review process, obtaining plans that are appropriate to their disability- related need.
  • Supporting clients to navigate the interface between NDIS, health, justice, education and housing, to ensure a holistic approach toward meeting need.
  • Participating in client conferences with other professionals, external agencies, and service providers.
  • Supporting clients to resolve points of crisis and develop resilience in their informal network.
  • Promote a positive team environment through the demonstration of good communication and interpersonal skills.
  • Ensure clear, accurate  records are kept and submitted on time.
