Aberstone Community Services is a Registered NDIS Provider that provides vital supports and services to people living with disabilities in the community. We are currently seeking applications for Part Time or Full Time Experienced Support Coordinators and Recovery Coaches to join our Support Coordination and Recovery Coaching team operating throughout the Perth metropolitan area. We are looking for experienced, passionate staff to support our participants living with physical, mental health, and other coexisting disabilities.
As a Support Coordinator or Recovery Coach you will work with our participants, their families, support networks, and cross organisational teams, to help them to achieve the most out of their NDIS plan. You will assist them to understand, implement and coordinate services for the length of their NDIS plan.
Preferred Candidate:
- Has an understanding of recovery orientated practice guidelines
- Will have a strong history in working with people with serious and persistent mental health conditions
- Has a self-directed passion and drive for the mental wellbeing sector and can provide capacity-building activities for the people they support
- A compassionate and collaborative approach to their work
- Enjoy finding innovative solutions to complex situations
Employee essentials:
- A tertiary qualification in peer work or mental health (minimum of Certificate IV in Mental Health Peer Work or Certificate IV in Mental Health) or equivalent training and/or;
- At least 2 years experience in a case management role working with people with a chronic and persistent mental illness, and working with their family;
- Experience within and understanding of relevant mainstream and community support services;
- Demonstrated ability to operate within organisational and legislative guidelines, procedures and policies;
- Build and sustain strong collaborative relationships with government and non-government bodies, service providers, allied health professionals, and practitioners.
- Ability to build participant’s capacity to self-direct, self-advocate, work with professionals and other supports, and manage their lives and their NDIS plan the way they choose.
- Ensure NDIS ethics and professionalism are maintained.
Compliance necessities:
- NDIS Screening Check, we can supply the employee endorsed code if needed
- Current Western Australia Driver’s Licence;
- Reliable and roadworthy motor vehicle to travel to customers’ homes and assist them to access the community, including comprehensive motor vehicle insurance and registration;
- Working with Children Card;
- Current First Aid Certificate
- Current CPR Certificate
- NDIS Worker Orientation Module certificate;
- Applicants must have the right to live and work in Australia
