Would you like to join the Metro North Health team and contribute towards achieving health equity for Aboriginal and Torres Strait Islander people?

Demonstrate accountability at an advanced practice level for managing and coordinating ongoing care for consumers/patients within Aboriginal and Torres Strait Islander Health Hub within Caboolture-Kilcoy and Woodford directorate.

Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.

About the role

  • The purpose of this role is to demonstrate accountability at an advanced practice level for managing and coordinating ongoing care for consumers/patients within Aboriginal and Torres Strait Islander Health Hub within Caboolture-Kilcoy and Woodford directorate and for the provision of specialist nursing advice, consultation, leadership, and education to inter-professional team members as relevant to the specific context of the role.
  • The role will:
  • Coordinate consumer/patient centred care across the consumer/patient journey.
  • Create partnerships, for example, providing a central point of communication, engagement and coordination for all stakeholders involved in the consumers/person's care.
  • Improve consumer/patient outcomes through the delivery of evidence-based care.
  • Facilitate systems improvement - providing a leadership role within the organisation and act as an agent of change.
  • Provide support to the Aboriginal and Torres Strait Islander Health Worker and Community Engagement Officer.
