ABOUT THE ROLE

•  Are you a self-motivated experienced and collaborative clinician with a focus on providing excellence in the clinical management of older people? 
•  Are you interested in ongoing research into the role of Nurse Practitioners in supporting people residing in Aged Care Facilities?

Hunter Primary Care is implementing an exciting Nurse Practitioner pilot program, providing in-reach clinics within participating residential Aged Care facilitates in the Tamworth region. 

The Aged CareNurse Practitioner role includes clinical leadership, assessment, diagnosis, therapeutic management and evaluation using nursing knowledge and skills which may include, but not limited to, the direct referral of consumers to other health care professionals, prescribing medications and ordering diagnostic interventions. 

With more than one position available, flexibility in working hours is available with the opportunity to be on a part-time or full-time basis. Relocation support is available on request, to make Tamworth your new home.

SKILLS & EXPERIENCE

  • Currently registered as a registered nurse with the Australian Health Professionals Regulation Agency (APHRA) and endorsed by the Nursing and Midwifery Board of Australia to practice as a Nurse Practitioner
  • Ability to deliver evidenced based, person-centered care within the Aged Care environment, work autonomously and collaboratively, using advanced and extended clinical skills
  • Demonstrated knowledge and understanding of regulatory compliance governing the Aged Care environment
  • Highly developed communication skills with a demonstrated ability to manage successful working relationships with residents, families, General Practitioners and the clinical services team.
  • Demonstrated effective time and schedule management to ensure residents expectations and health care goals are met
  • Evidence of full vaccination against COVID-19
  • Current NDIS Worker Screening Check clearance
  • Recent or new (employer facilitated) Criminal History Check.  Please note that people with criminal records are not automatically barred from applying for this position.  Each application will be considered on its merits.
