The Opportunity:
We are on the lookout for seasoned Nurse Practitioners to bolster our wellness hub teams across NSW, VIC, QLD, and SA. This role is pivotal in conducting clinical assessments, orchestrating care interventions, and liaising with allied health professionals to ensure top-notch care for our residents. While the majority of work will be conducted via telehealth, we aim for Nurse Practitioners to be situated near their respective hubs, allowing for in-person visits as needed.
Key Responsibilities:
- Perform thorough clinical reviews and assessments.
- Work collaboratively with residents, their families, and health professionals to customize care plans.
- Facilitate telehealth consultations while accurately maintaining electronic health records.
- Play a role in research and initiatives aimed at quality improvement.
- Offer guidance and education to healthcare professionals and care staff.
Who You Are:
- Holder of a Bachelor and Master of Nursing (Nurse Practitioner) degrees or equivalent.
- Currently registered as a Nurse Practitioner with the Nursing and Midwifery Board of Australia (NMBA).
- Preferably experienced in aged or primary care settings.
- Possess excellent communication skills and the ability to operate both autonomously and as part of a team.
- Well-versed in Aged Care Quality Standards and dedicated to evidence-based practice.
