The Aged CareNurse Practitioner/TNP outreaches into the Aged Care facilities in Bathurst offering specialised advanced clinical care with a hospital avoidance focus, aiming to reduce the number of emergency presentations, inpatient admissions and length of stay. The role is an integral part of the multidisciplinary health care team liaising with ED, Palliative Care and the inpatient Aged Care team.
