Job Description
Works within the care coordination framework and provides clinical Occupational Therapy services including individual assessment and interventions and family/carer interventions.

As care co-ordinator completes all mental health state-wide assessment and documentation requirements including mental state examination, risk assessment, outcome measures, crisis plans and utilises working knowledge of use and processes of the Mental Health Act 2014.

Contributes to clients individualised treatment, support and discharge plans including                                     targeted occupational goals which are formulated with the participation of clients and their carers/families (unless clinically inappropriate).

Contributes to the development and evaluation of the community Occupational Therapy service working with the Senior Occupational Therapist, multi-disciplinary team and other stakeholders.

This is a recruitment pool for permanent full time, permanent part time, fixed term full time, and fixed term part time appointments.

Suitable applicants will be placed in a pool from which appointments may be made when similar vacancies occur. Appointments may be made from this pool until 30/08/2024.
