Your day to day will include:

• Collaborating with a range of stakeholders in service delivery to ensure clients are provided the support & skills they need to participate in their community.
• Service clients in a limited radius near your location around Melbourne's Western suburbs
• Ensure customers receive tailored services that are individualised & outcomes focused
• Apply a client centric model to develop, implement and review Individual Support, Wellness or Recovery plans with clients
• Conducts relevant therapeutic assessment with clients developing the resulting reports & plans
• Deliver a variety of one-on-one or group based activities in a range of settings including onsite and in the community
• Record accurate and timely notes & billing relating to activities & outcomes for clients
• Meet billable hours targets of 5 hours per day(includes report writing, face to face and travel)

Requirements

• Tertiary qualification in Occupational Therapy
• AHPRA registration
• Unrestricted working rights in Australia 
• Drivers licence and own vehicle
