This is a multidiciplinary role and remuneration will be based on qualifications and experience
Role Description
The Continuing CareOperations Manager is responsible for effective leadership, management, direction, support and development of staff within the following programs:
- Community Dental Services
- Outpatient Rehabilitation Service (OPRS)
- Transition Care Program (TCP) /Geriatric Evaluation and Management at Home/ Residential In-Reach (RIR) / Rehabilitation in the Home (RITH)
- Hospital in the Home (HITH) / Post-Acute Care (PAC) / Hospital Admission Risk Program (HARP) / Diabetes Education
- Referral Centre
- Better@Home project
The successful applicant will be subject to the satisfactory completion of Credentialing and Scope of Practice at Bendigo Health.
