Overview
The Team LeaderOptimal Ageing is an experienced and highly competent worker who coordinates quality services and provides operational leadership to a multi-disciplinary team of health professionals. The Team Leader undertakes operational management including monitoring of service provision, data collection and service evaluation for community-based services. BHFLHN Community & Allied Health Directorate has teams of health professionals whose role it is to provide services in the community, in the acute sector and through a variety of service models. The Team leader is responsible for contributing to improvements in the quality, safety and community needs-driven distribution of services across BHFLHN. The Team Leader works with nursing, allied health and other community health staff, program coordinators, managers and other stakeholders on models of care and workforce development initiatives. As a member of the extended Community Health leadership team, the Team Leader will contribute to operational planning for the program and ensure that all services are integrated with other key programs.
