The Derbarl Yerrigan Health Service (DYHS) was established in 1974 and is the largest Aboriginal community-controlled health service provider in Western Australia, providing care to over 20,000 Aboriginal patients across four clinics in East Perth, Mirrabooka, Maddington and Midland. Our staff enable the delivery of culturally responsive primary health services, focusing on prevention, early intervention, comprehensive care and care coordination including chronic disease management.

About the role

The Paediatric Case Officer is responsible for coordinating and managing specialist paediatric clinics, arranging appointments with various specialists, and acting as the key worker for children engaged with the clinic. This role is pivotal in ensuring that all aspects of care are managed effectively in accordance with Derbarl paediatric model of care.

This role will demonstrate a high level of cultural sensitivity and awareness. This includes an understanding and respect for the diverse cultures with particular emphasis on Aboriginal communities. The role will ensure that all practices and initiatives are conducted in a culturally inclusive manner that respects the traditions and lands of Aboriginal people. This role requires ongoing commitment to cultural competence training and development to enhance Derbarl practices and alignment with principles of equity and inclusivity.

Key Responsibilities

1. Clinic Coordination and Case Management

  • Schedule and coordinate appointments with various specialists such as general and developmental paediatricians, dermatologists, audiologists, ENT specialists and respiratory specialists.
  • Act as the primary liaison between patients, families, and healthcare providers ensuring seamless communication and continuity of care.

Qualifications, Skills and Experience

  • Bachelor's degree in nursing, Social Work or a related health field (Desirable)
  • Proven experience in paediatric healthcare settings.
  • 2 years' experience working with Aboriginal/Torres Strait Islander children and families or an understanding and appreciation of Aboriginal culture.
  • Demonstrated understanding of the complexity of Aboriginal Child and Maternal Health issues.
  • Experience in case management and care coordination of clients with complex and chronic medical and developmental needs.
  • Familiarity with medical terminology and paediatric healthcare services.
  • Strong organisational skills and the ability to manage multiple tasks simultaneously.
  • Excellent communication and interpersonal skills.
  • Proficiency in using healthcare databases particularly Communicare.
  • Strong organisational skills with the ability to multitask and prioritise effectively.

Statutory Requirements

  • Aboriginal and/or Torres Strait Islander pursuant to Section 50D of the Equal Opportunity Act
  • Must have valid Work with Children Check
  • Recent Police Clearance certificate
  • Valid AHPRA registration (Desirable)
