Key Responsibilities include:
- Deliver general level clinical Pharmacy Services within a rostered clinical area or in the dispensary as part of a multidisciplinary team in accordance with prescribed professional and ethical standards and under the direction of the Senior Pharmacist (Operational Portfolios).
- Provide accurate and timely advice to medical, nursing and subordinate pharmacy staff on patient admission history and pharmaceutical issues while ensuring optimum patient medication safety.
- Undertake routine professional pharmacy tasks (including taking a medication history upon admission, inpatient chart review, medication action planning, medication discharge planning, provision of discharge medication record, dispensing and counselling, liaison with community providers, medication safety improvement initiatives, periodic audits of prescribing, supply and controlled drug records, and other agreed key performance indicators) within established protocols.
- Provide input in the development of clinical practices, procedures and protocols within the Pharmacy Services work area and participate in quality and service improvement activities to continually improve patient / client care.
