Role Overview:
As a Practice Nurse, you will play a vital role in delivering high-quality patient care within a supportive and collaborative environment. Your responsibilities will include providing clinical support to GPs, managing chronic disease programs, performing health assessments, and promoting preventative health measures. This is an excellent opportunity for a skilled nurse looking to make a significant impact on community health.
Key Responsibilities:
- Provide comprehensive nursing care to patients of all ages.
- Assist GPs with clinical procedures and minor surgeries.
- Conduct health assessments, immunizations, and wound care.
- Manage chronic disease management plans.
- Educate patients on preventative health and lifestyle choices.
- Perform ECGs, spirometry, and other diagnostic tests.
- Maintain accurate patient records and documentation.
- Collaborate with healthcare professionals to deliver coordinated care.
Qualifications and Skills:
- Current registration with AHPRA as a Registered Nurse (RN).
- Current CPR / First Aid certification.
- Minimum of 2 years of nursing experience, preferably in a general practice setting.
- Strong clinical skills and knowledge of chronic disease management.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in using medical software and electronic health records.
- Commitment to continuous professional development.
