We are looking for an RN who has experience in primary care and is keen to shape patient care and improve outcomes.
You will be a valued part of our established multidisciplinary team, consisting of one other experienced RN, GPs and allied health professionals who are all focused on providing excellent care in a warm, supportive environment.
The Practice is digitally enabled and community orientated with a focus on the present and an eye on the future of primary care.
Your role as our Practice Nurse will focus on enhancing patient outcomes, delivering care in practice and virtually and collaborating on a regular basis with other team members and patients.
Ideal strengths
- Ability to co-ordinate patient care and manage patient data
- Comfortable in assessment of acute patient presentations
- Strong communicator
- 12 months experience as a Practice Nurse
- Accredited Nurse Immuniser
- Team player
