About the Role
The Registered Nurse (RN) in the Rehab@Home team delivers safe compassionate clinical care; working collaboratively with the healthcare team to achieve best practice patient outcomes. The RN engages in reflective, analytical practice through effective assessment, planning, implementation and evaluation of direct person-centred care. The RN continually improves their skills and knowledge developing from graduate to highly skilled Registered Nurse.
This role works directly with nursing and allied health as part of a multidisciplinary team to provide care to clients in the community. This role works independently within the healthcare team structure and with the support of senior clinicians to provide slow stream restorative care and is pivotal to the planning and delivering of community nursing care which may include wound management, catheter care, medication administration and client and family education on self-care and management.
