Registered Nurse - Cardiac Outpatient Rehabilitation

Epworth Geelong offers a range of multidisciplinary Outpatient Rehabilitation programs.

We are seeking a Registered Nurse (RN) with experience in cardiac nursing and/or chronic disease management to compliment and support the Cardiac Outpatient program. 

Joining our Allied Health outpatient team you will contribute to the provision of coordinated, high quality Cardiac Rehabilitation services and the flow of care from inpatients to outpatients.

Key responsibilities will include:

• Delivering continuity of care to patients through their care journey at Epworth Geelong
• Liaising with Cardiologists to promote referral to outpatient services at Epworth Geelong
• Cardiac patient nursing assessments
• Delivery of nursing education to the cardiac rehabilitation cohort of patients

To be successful you will have:

• Be a Registered Nurse with current AHPRA registration
• Be available for a minimum of six hours on Wednesday
• Enjoy facilitating the very best outcomes for patients and families, from admission to discharge
• Add value in a dynamic multidisciplinary team environment and collaborate with like specialties across the Epworth Healthcare Group
• Be experienced in the provision of cardiac rehabilitation education
• Present with highly developed communication & interpersonal skills and an ability to build effective relationships
• Demonstrate high levels of self-motivation and accountability and a willingness to work within agreed parameters
• Have previous experience in a leadership role
