About us:

The Nelson Bay Community Nursing team is made of two speciality areas- Community Nursing and Palliative Care. The small team are located within the campus of Tomaree Community Hospital and Health Centre – 5 mins away from Shoal Bay Beach.

The collaborative team cover two areas within Port Stephens- Community Nursing is within the Tomaree Peninsula and Palliative Care, is a larger area covering a large portion of the Port Stephens LGA.

About the job:

  • In this role you will relieve Nurses in each team when on leave. A typical week may be in Community Nursing or Palliative Care- pending on leave of the team. This means the Nurse may be driving out and about within the LGA providing symptom control to our Palliative Care clients or running a wound care clinic in the Health Centre for our Community Nursing clients. There is certainly never a dull moment with the two specialities caring for clients across a large demographic area.
  • If you are a person who likes to learn new skills and after a changeable role then this role is ideal for you.
  • Whilst made up of two teams, the Nurses have a fabulous collegial relationship and assist each other in day-to-day tasks. The team works together to achieve positive patient outcomes and get great satisfaction out of providing excellent client care.
  • This role is a 7-day roster position with 4-hour shifts on a Saturday and Sunday (when required) for Community Nursing and 8-hour shifts on a Saturday and Sunday for Palliative Care. Public holidays are also as required.
  • Our successful Nurse will develop advanced skills in Wound Care, Continence, Community Health, Aged Care, CVAD/Medication Management, Caseload Coordination and Specialist Palliative Care. A comprehensive training program would begin initially with Community Nursing and then into Palliative Care depending on current skill set.
  • Community Nursing holds regular education forums and have a transition to a speciality programme and clinical skills training with Nurse Practitioners.
  • Palliative Care has education and training delivered via the District Palliative Care stream as well as a face-to-face orientation programme.
  • Professional development via external education is also supported (where directly relevant).

Requirements:

Our successful candidate will possess the following:

  • Current registration as a Registered Nurse with AHPRA.
  • Our successful candidate will be flexible as their role varies from week to week- for example they may be working in Community Nursing for one week and Palliative Care the next. You will therefore be able to work weekends and provide the on call after hours telephone support that the Palliative Care Team operates for all Palliative Care clients. Flexibility is a non-negotiable requirement for this role.
  • Ideally you will have experience in both areas of Nursing within the community and will demonstrate experience in caseload coordination, teamwork, and excellent communication skills. To undertake this role well the ideal candidate will be eager to develop new skills and enjoy the challenge of learning and developing two nursing speciality areas.
  • An ideal candidate will demonstrate outstanding teamwork skills as they are needed to work alongside and support their colleagues in both teams.
