About the role:
As a Registered Nurse specializing in Home and Community Care, you will collaborate with interdisciplinary teams, clients, and their families to assess, plan, implement, and evaluate holistic care plans tailored to meet the unique needs of each client. Your primary focus will be on promoting health maintenance, disease prevention, and providing skilled nursing care to optimise clients' independence and quality of life in their home environments.
Responsibilities:
- Implement and uphold clinical care and services according to governance and operational requirements.
- Prioritize clients in decision-making, considering their needs, choices, independence, dignity, and risks.
- Complete client evaluations accurately and seek improvement opportunities when needed.
- Collaborate with clinical teams and external providers to ensure optimal client outcomes.
- Document client incidents, analyze data, and propose strategies for prevention and correction.
- Monitor and address client skin integrity, medication, nutrition, hydration, and behavior.
- Engage in client review conferences and involve families/carers in care delivery.
What we look for:
- Registered Nurse DIV 1 - AHPRA Registered.
- 2+ years previous experience in Aged Care
- Proficient computer skills and working knowledge of iCare (desirable)
- Understanding of WHS responsibilities, including manual handling principles
- Knowledge of Aged Care Legislation, Accreditation and ACFI will be highly regarded
- Current First Aid Certificate, Valid police check clearance
- Current flu and Covid vaccination
- Must be eligible to work in Australia.
