About the role
As the RN/Care Manager for our client you will be working in the community supporting clients in their own home, allowing them to stay independent for as long as possible. You will provide comprehensive assessments and develop care plans based on the assessments to support the clients.
You will be empowered to contribute to the ongoing development of the program, lead clinical governance within the service and provide feedback to managers to identify staff needs and achievements.
