Think Continence Pty Ltd is a specialised clinic located in Cairns, dedicated to providing expert care in bladder, bowel, and wound management. Delivery of services is via Home visiting, Tele-health and Clinic based services. The team offers personalised services to a diverse range of clients: NDIS Self and Participant Managed Clients: Supporting both children and adults who are part of the National Disability Insurance Scheme (NDIS). Aged Care Clients: Participants of home care packages receive tailored assistance to maintain independence in their local community. Private Clients requiring nursing We are a small, highly driven team who want to make a difference by using our nursing skills to change lives.
Qualifications & experience
- Minimum of 2 years full time experience in a hospital role is highly desired, and/or the applicant has disability, aged care and community nursing experience.
- Community nursing interest desirable
- First Aid/CPR certificate mandatory
- Working with Children Check
- NDIS screening check clearance (Disability Yellow Card) mandatory
- National Police Clearance necessary
- AHPRA registration mandatory
- Post Graduate Certificate in the relevant field of Continence, Wound management, stomal therapy or other relevant course, or ongoing studies would be highly desirable
- Drivers licence and car in good working order
Tasks & responsibilities
- The Registered Nurse will be responsible for person-centred care met through effective assessing, planning, implementing, and evaluating clinical care for NDIS and Aged Care Clients in the Cairns and surrounding areas. The focus will be on continence, wounds and stomal therapy.
- Responsible for providing safe, compassionate care, integrating Nursing Services by caring for each client's psychological, social, emotional, spiritual, and physical care needs to achieve best clinical practice and individualised client care and outcomes.
- Contributes to quality care through ongoing learning and development, preceptorship, mentorship, role modelling and shares information with peers, and work colleagues.
- Educates, informs, counsels and supports clients, their families, carers, support coordinator and support workers on general and clinical information relevant to client's care seeking expert advice as required.
- Supports the Think Continence Team and demonstrates a strong team commitment through open communication and active participation in the development and achievement of the organisation's goals
- Demonstrates motivation, effective change management, self-awareness and commitment to positively contribute to the workplace culture.
- Demonstrates a high level of literacy and the ability to formulate nursing care plans, obtain equipment quotes and deliver detailed written reports based on a client's continence needs.
- Utilises critical thinking skills, flexibility and time management skills to plan and prioritise client care, adapts to changes and coordinates referrals.
- Part of the role is home visiting clients in a geographic range including Port Douglas, Mareeba and Atherton Tablelands and Innisfail. For this reason the applicant must be willing to supply their own car (compensated by travel allowances).
