About the role:
Our successful candidate will be responsible for the co-ordination and application of entry into aged care facilities on behalf of the patients, families and carers of patients in STRAS.
Support will be provided by the Nurse Unit Manager and work alongside and under the guidance of our experienced placement co-ordinator.

• Belmont Hospital offers a unique opportunity to make a difference. An enhanced education team including after-hours education support with increased Nursing Hours Per Patient Per Day (6NHPPD) enabling excellence in healthcare delivery and positive outcomes for patients and staff.
• STRAS is a 20 bed ward established to co-ordinate admission into residential aged care following an acute hospital episode.
• Our goal is to provide temporary care whilst actively involving the family, patient and/or carer to find a suitable facility to meet our patient’s needs.

We recognise this transition from home into aged care is often a difficult and unsettling time for families, and we provide a friendly, professional service to make this transition as seamless as possible.

About you:
Our successful candidates will:

• Demonstrate the ability to provide compassionate care for high-risk patients experiencing confusion and delirium.
• Implement evidence-based practice guidelines to minimise risk when caring for frail and aged patients with complex care needs.
• Work effectively within their scope of practice to achieve optimal patient outcomes.

Information for Applicants:
All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA).
