About the role:
Our successful candidate will be responsible for the co-ordination and application of entry into aged care facilities on behalf of the patients, families and carers of patients in STRAS.
Support will be provided by the Nurse Unit Manager and work alongside and under the guidance of our experienced placement co-ordinator.

- STRAS is a 20 bed ward established to co-ordinate admission into residential aged care following an acute hospital episode.
- Our goal is to provide temporary care whilst actively involving the family, patient and/or carer to find a suitable facility to meet our patient’s needs.

Our successful candidates will:
- Demonstrate the ability to provide compassionate care for high-risk patients experiencing confusion and delirium.
- Implement evidence-based practice guidelines to minimise risk when caring for frail and aged patients with complex care needs.
- Work effectively within their scope of practice to achieve optimal patient outcomes.
- Four weeks annual leave (pro-rata for part-time employees)
- Up to 12 allocated days off each year (for full-time employees)
- Superannuation contributions 11%

Information for Applicants:
All NSW Health workers are required to have completed a primary course of a COVID-19 vaccine which has been approved or recognised by the Therapeutics Goods Administration (TGA). Additionally, Category A workers are required to receive a booster dose three months after completing the primary course of COVID-19 vaccinations. Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
