Transitional Aged Care Program (TACP)
Transitional Aged Care is a short term (up to 12 weeks) Commonwealth funded aged care service for older people leaving hospital who would usually be eligible for ongoing care in a residential aged care facility but who have the potential to improve their physical, cognitive or emotional function and wish to try and remain in their home.
An opportunity exists to work with a dedicated multi-disciplinary team of TACP staff, ACAT Assessors, Dementia support, psychogeriatric Service, Podiatrists, ASET team, occupational Therapy, Speech pathology & Physiotherapy Service, Discharge Planner & AARCS Nurse.
Within this position you will deliver quality, efficient and effective person centred care which meets the needs of the patients and their families or carers and act as an advocate to assist patients and their families/carers make informed decisions. You will share in leading the team of EENs with other RNs in providing an efficient and effective TACP Service including timely response to referrals for intake.
You are a confident, friendly team player who will be able to work well within the cohesive team and will demonstrate compassion to our clients.
As the successful applicant you will be supported with the appropriate training to be an effective TACP RN as well as additional support once established in the role, if required.

COVID-19 booster doses are highly recommended for all other health care workers who have completed the primary course of COVID-19 vaccinations.
