About the role
Lismore Community Health has an exciting opportunity to work in a supportive, motivated and professional team providing social work to clients of the Transitional Aged Care Program in the Ballina and Lismore areas. Providing comprehensive and appropriate Social Work assessment and intervention as an integral part of the multidisciplinary health care team for patients/clients of the Northern NSW Local Health District (NNSWLHD).
