Position Title: Social Worker (Transitional Aged Care Program)

Employment Type: 2 x roles (Permanent Full Time and Maximum Term Part Time)

Hours per week: 38 hours and 24 hours per week available

Position Grade: Social Worker Level 1/2

Salary: $70,944.00 - $101,211.00 base salary per annum (pro rata for part time)

Enterprise Agreement: The Named NSW (Non-Declared) Affiliated Health Organisations' Professional & Associated Staff Agreement 2022

About the role:

We have an exciting opportunity to work in a dynamic team leading our approach to the provision of quality care to clients of the TACP Department.

This role works within a multidisciplinary team to provide Social Work assessment and intervention for clients in the community on the Transitional Aged Care Program and includes Case Management of clients as they progress through the 8-week program.

This role is involved in quality improvement projects within the Transitional Aged Care Team, liaises with community services/groups and health professionals to assist clients to achieve their TACP goals, and contributes to the education of the Community Aged Care Team and other community teams as required.

Selection Criteria:

  1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent's Health Australia.

  2. Evidence of Tertiary Qualification in Social Work and eligible for membership of the Australian Association of Social Workers (AASW)

  3. Evidence of clinical experience in a hospital or health related field as a practising Social Worker.

  4. Current NSW drivers' license Class C or above.

  5. Demonstrated ability to work autonomously and as part of a multidisciplinary team.

  6. Demonstrated effective communication, consultation, and interpersonal skills.

  7. Demonstrated commitment to improving clinical practice and service delivery through leadership of quality improvement activities and ongoing professional development.
