Registered Nurse
You Connect (YC) is a not-for-profit organisation that supports people living with a disability, have complex behaviours and/or have mental health diagnosis that impact their day-to-day functioning.
We support people, from age 6, by providing a range of services in capacity-building supports, community participation, employment readiness & housing.
We are looking for a dynamic and energetic person to join our Disabilities and Mental Health division. This is a flexible role reporting directly to the CEO, working alongside our Disabilities and Mental Health Operations Managers whilst supporting our Central Coast and Newcastle teams.
Requirements
Along with a current AHPRA registration and minimum 2 years’ experience in Community Health or Disability/or disability sectors, you have:
- Comprehensive knowledge of consumer directed and self-directed care models
- Demonstrated ability to partner with unregistered care staff to build their skills and knowledge to improve client outcomes
- Demonstrated understanding of contemporary community and/or disability clinical practice
- Outstanding knowledge of the needs of people with a disability
- Strong time-management skills including managing multiple deadlines
- Understanding of and commitment to business excellence
Duties and Responsibilities:
- Deliver quality clinical nursing care services in line with client care programs and service plans in response to emerging clinical client needs, with clinical assessments, and associated documentation completed within the required timeframes.
- Provide advice and support to staff in relation to any support required by clients, working with staff and clients to identify solutions that meet client needs and promote client independence.
- Undertake clinical assessments, contribute to the development of client plans, and evaluate client plans to be in line with dynamic client needs
- Facilitate communication with staff, clients, and all necessary stakeholders to ensure appropriate case conferencing and goal attainment is achieved
- Collaborate continuously with staff by providing support where necessary, delegating care activities, and working together to implement clinical auditing processes.
