About the Role
The role involves working closely with the multidisciplinary team to co-ordinate care for these patient groups, including assessment, interventions, referrals and discharge. There is no on-call or after-hours work related to this position.
YACDS support young people with complex disabilities (predominantly, but not exclusively Cerebral Palsy) to transition from the care of the Royal Children's Hospital into adult healthcare settings. The role involves outpatient multidisciplinary and medical only clinics, with case reviews and Spasticity management.
The position is 8 hours per week, on Tuesday.
- Comprehensive evidence based medical assessment and management of YACDS clients.
- Clear and timely communication with the client's General Practitioner, family and/or carer as relevant
- Participation in the development and maintenance of high-quality service by implementing incremental improvements to systems, processes and patient care.
- Achievement of HIP quality and activity targets including waiting list targets within the allocated financial budget
Classifications for this position will be dependent on number of years of Specialist Experience.
Your Contribution
- Specialist registration with the Medical Practitioners Board of Australia / AHPRA
- Fellowship of the Australasian Faculty of Rehabilitation Medicine or equivalent specialist qualification
- Participation in an approved CME program
- Applicants must be Consultant Rehabilitation Physicians with the approval to provide Medicare services in a metropolitan public hospital.

About The Health Independence Programs (HIP) Pain and Disability Stream
HIP was introduced by the DH&HS in 2008 bringing together the four previously separate service streams of Subacute Ambulatory Care, Post Acute Care, Hospital Admission Risk Program and Residential In Reach under common guidelines. The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centered care planning.
These services provide specialised multidisciplinary outpatient care to patients from across Victoria.
