About the Opportunity
An exceptional opportunity has arisen to join Felicity Holistic Care's Residential team as a Care Coordinator. This role is dynamic and fast-paced and is fundamental to the success of our business and to providing health and happiness to our clients. The team has an exceptional culture and aspires to be solutions focussed. You will be involved in a wide range of exciting projects including the implementation of new systems and processes, whilst working closely with our high-performing team leaders.
Your key responsibilities will include:
- Providing exceptional customer service outcomes
- Managing the coordination of our Support Workers to deliver disability services in line with policies and procedures
- Being the primary point of contact for our Residential support staff and clients
- Having responsibility for building rapport and engaging with our casual and permanent workforce
- Providing advice and support for any immediate crisis situations and liaise with; clients, emergency services and support workers to achieve optimum outcomes
- Ensuring that all administrative tasks are completed to the required standard and key performance indicators are met
- Completing reports regarding client issues and managing outcomes
- Ensuring all Care Coordination practice is at the highest quality standard, taking measures to improve processes and methods when possible and when required
- Regularly checking that care coordination practice and documentation within the organisation is compliant and taking action to maintain it.
To be successful in this role you will bring:
- Experience planning and leading high-quality support services, and have a positive solution-focused attitude and approach
- ·An open-minded approach to supporting children and adults with complex needs, developmental delays and various cultural backgrounds
- Strong problem-solving and decision-making skills
- An ability to work well under pressure
- High-level decision-making and critical thinking skills
- Experience and skills in leading high-performing teams, including conflict resolution and performance management
- Lead by example and have the ability to support and upskill individuals to gain confidence in their personal care abilities
- Proven experience working within a Residential Care environment
- Demonstrated experience in operational aspects of disability support service delivery including knowledge of compliance, standards and processes
