As a Home and Community Care worker, you will build quality relationships with elderly individuals and NDIS participants of all ages.
We are seeking casual Clinical Nurses with an understanding of the Aged Care Quality Standards and Home Care Packages to will complete clinical assessments and assist with providing support, and encouragement of independence to reside in their chosen community.

Mandatory Requirements:
- Current AHPRA registration
- Current National Police Check
- Current CPR certificate
- Current Flu Vaccination
- Current Covid vaccination and state government booster requirements
- Current paid employee Yellow Card/NDIS Worker Clearance.
- Smart phone with internet access
- Two current (within the past 2 years) professional references. One of those references will be required from a Supervisor/ Management role.
Responsibilities:
- Liaise with clients and/or their representatives on clinical aspects of their Home Care Package
- Manage and create risk profiles and action plans.
- Provide clinical guidance to the organisation.
- Deliver clinical education and recommendations to consumers where a health concern or risk has been identified.
- Respond to escalated health concerns of our consumers, including liaising with consumers and/or their representatives about clinical management plans.
- Collaborate with the clinical team to determine the suitability of consumers to self-manage their Home Care package.
Skills & Attributes:
- Relevant understanding and experience in the delivery of health services, chronic disease management, and complex case management.
- Practical understanding of the Aged Care Sector, Quality Standards, and Principles. Home Care Package program knowledge and experience is highly desirable.
- Confident and skilled in supporting and working with clients over the telephone and in person.
- Reliable, hardworking, determined, and self-motivated.
- Strong interpersonal and communication skills.
- Well organised, able to prioritise, and meet competing deadlines.
- Excellent attention to detail, able to problem solve and make decisions based on consumers requirements.
- Relevant understanding and experience in the documentation and clinical assessments.
