Regularly reviewing the data collected, you'll closely monitor patients' health status, identifying any concerning trends or deviations that may require intervention. Collaborating closely with other healthcare professionals, you'll ensure timely adjustments to treatment plans and provide personalized guidance to patients based on their remote monitoring data. Your support and encouragement will be pivotal in fostering patient engagement and adherence to monitoring protocols, ultimately enhancing their overall well-being and quality of life.People of Aboriginal and/or Torres Strait Islander background encouraged to apply.
Candidates will need to meet the following criteria:
- Consistently demonstrates behaviours that reinforce the CORE Values of our organisation; Collaboration, Openness, Respect and Empowerment. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.
- Current registration as a Registered Nurse with the Australian Nurse Health Practitioner Regulation Authority (AHPRA) with relevant post-registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualification or such other qualifications or relevant equivalent experience.
- Demonstrated experience in leading and initiating activities to improve clinical service delivery and patient clinical outcomes.
- Technical proficiency, using remote patient monitoring technology platforms and software, troubleshooting technical issues, and data analysis.
