A unique opportunity exists for an experienced cardiology nurse to support and empower patients to remotely monitor Heart Failure signs and symptoms in the community.

Where you'll be working


North Sydney Local Health District, Management of Cardiac Function (MACARF) – based at Royal North Shore Hospital

What you'll be doing

The NSLHD Nursing and Midwifery Workforce will be compassionate, competent, confident, contemporary, capable and committed to leading person-centred health care.

You'll be responsible for educating patients on the proper use of remote monitoring devices, empowering them to track vital signs and symptoms from the comfort of their homes. Regularly reviewing the data collected, you'll closely monitor patients' health status, identifying any concerning trends or deviations that may require intervention. Collaborating closely with other healthcare professionals, you'll ensure timely adjustments to treatment plans and provide personalized guidance to patients based on their remote monitoring data. Your support and encouragement will be pivotal in fostering patient engagement and adherence to monitoring protocols, ultimately enhancing their overall well-being and quality of life.

  2. Current registration as a Registered Nurse with the Australian Nurse Health Practitioner Regulation Authority (AHPRA) with relevant post-registration qualifications and at least 3 years' experience working in the clinical area of their specified post-graduate qualification or such other qualifications or relevant equivalent experience.
  3. Demonstrated experience in leading and initiating activities to improve clinical service delivery and patient clinical outcomes.
  4. Technical proficiency, using remote patient monitoring technology platforms and software, troubleshooting technical issues, and data analysis.
