Position Profile: The occupant of this position will help to ensure Aboriginal people approaching the end of their lives, and their families, have access to appropriate palliative and end-of-life support across the continuum of care from diagnosis of a life limiting illness to death and into bereavement. You will provide cultural advice the multidisciplinary team regarding cultural issues impacting patient care. You will support patients and family's access and referral to services as needed, ensuring respectful and appropriate palliative care for Aboriginal people. You will provide supervision and support to other Aboriginal Health Workers and provide supervision and support to other Aboriginal Health Workers.
