About You
SWSLHD is seeking an experienced Physiotherapist to join the community-based Liverpool Transitional Aged Care Program (TACP).
This is an excellent opportunity for an experienced physiotherapist who has extensive knowledge and experience working with a range of Aged Care and rehabilitation populations.
As a Senior Physiotherapist you will require strong clinical reasoning across a diverse range of clinical areas and will have excellent communication and problem-solving skills. You will have demonstrated leadership capacity and be dedicated to achieving improved functional outcomes for older people transitioning back into the community.
About the Opportunity
TACP is a unique program focused on the reablement of older adults living in the community. It is a multi-disciplinary, community-based, program that aims to deliver better health outcomes and maximised quality of life for individuals transitioning into the community following a hospital admission. You will work closely with the multi-disciplinary team and other relevant stakeholders to ensure holistic care for TACP clients and their families.
We are currently seeking to fill a full-time permanent level 3 physiotherapist role within the Liverpool Transitional Aged Care Program. The office is located within Braeside Hospital and is based within a multi-disciplinary team.
In this role you will be responsible for the assessment of TACP clients in their homes, holistic goal setting, development of client centered treatment plans and discharge planning. You will be committed to evidence-based practice and quality improvement and will lead initiatives in these within the physiotherapy team.
As a Senior Physiotherapist you will also be responsible for the ongoing education, training, and supervision of level 1 / 2 physiotherapists, allied health assistants and students. Alongside a multi-disciplinary senior team, you will also have the opportunity to be involved in guideline and policy review, education and training of staff, and district wide QI/EBP projects.
What You'll be Doing
This position is responsible for providing high quality physiotherapy services. This includes service development and evaluation and advanced reasoning and clinical practice skills in the management of clients of a novel or complex nature. This position is responsible for the supervision, teaching and training of Level 1/2 Physiotherapists, AHAs and students.

How to Apply
To be considered for this position, please ensure you address the selection criteria as thoroughly as possible. * Current Physiotherapy Registration with the Australian Health Practitioner Regulation Agency (AHPRA)

  • Relevant clinical experience including demonstrated advanced practice skills and management of a complex physiotherapy caseload.
  • Demonstrated experience in supervision and teaching of staff and/or students.
  • Demonstrated commitment to professional development and evidence based practice.
  • Demonstrated knowledge and experience in quality improvement, research and service development activities.
  • Demonstrated effective interpersonal and verbal and written communication skills.
  • Demonstrated effective time management skills and ability to work independently and as part of a multidisciplinary team.
  • Extensive clinical knowledge in Aged Care and/or rehabilitation physiotherapy with a broad range of clinical experience including acute / subacute / community.
  • Current unrestricted Australian drivers licence (P2 Licence Acceptable) subject to obtaining NSW drivers licence within 3 months of appointment.
