About the role
This position isresponsible for developing Avivo's Community Teams in Rockingham / Mandurah tosupport customers to live a good life within their community. The ServiceCoordinator works in partnership with customers to design and develop supportsto enable them to:
- Lead fulfilling lives in a way that makes sense to them
- Achieve their goals and outcomes
- Maintain their independence
- Be a contributor and valued member of their community
- Maximise the use of their funding
The ServiceCoordinator is responsible for ensuring quality support and services areprovided to customers within the area and sourcing resources needed. They workclosely with teams, supporting them to provide excellent services to customersand ensuring they have the knowledge, skills, values, and resources required.
This position works inpartnership with colleagues within the region and across Avivo to developknowledge and skills in working with particular funders and target groups,i.e., disability, aged care and/or mental health. They contribute to thecommunities of practice that focus on the above mentioned.
Duties include
- Leadership, recruitment, and development of the team offield-based staff
- Planning and providing services against budget within theirgeographical area
- Supporting the team with knowledge and skill
- Working within a person-centred framework to ensure supportis provided in the most efficient way possible by working closely with theMentors, Team Coach, Service Coordinator Team, Resource Team, Team Facilitatorsand Support Workers.
Selection criteria
- Previous experience in a similar role of 2 years or more(essential)
- Certificate IV in Disability, Aged Care, Mental Health (or workingtowards)
- Previous experience in supporting and developing employeesand teams
- Demonstrates ability to work as a supportive leader, withability to coach and resource others' development
- Knowledge and understanding of disability, aged care and/ormental health sectors and demonstrates contemporary professional knowledge,values, and practice
- Commitment and skills in providing excellent support tocustomers
- Understanding and commitment to working with communities towelcome and include all people as valued citizens
- Excellent interpersonal and communication skills, numeracy,and literacy
- Strong IT skills including ability to use portals, systems,and document relevant information
