Days & Hours of Work: Monday - Friday totalling 38 hours per week
Competitive Salary: Band 5 - $78,462.47 p.a + 11% superannuation

In this role, you will be responsible for supporting the Social Support and Community Transport Team Leader by:

  • Researching, planning, developing a range of interest-based activities and outings that engage and connect with the needs of our clients.
  • Creating and reviewing service-specific support plans in response to the clients and their carer's changing needs and aspirations.
  • Working collaboratively with the team on delivering programs and activities that stimulate and enhance participants' wellbeing.
  • Supporting the completion and prioritisation of assessment service referrals.
  • Supporting and undertaking a diverse range of administration tasks that contribute to a high-quality customer experience.
Key Selection Criteria:
  • Passion for working with older people to enhance their social connectedness in the local community.
  • Tertiary qualification in a management, health, allied health, social science, or related discipline and/or significant experience working with older people in a community setting.
  • Knowledge and understanding of aged care programs as well as the needs and issues affecting older people and their carers.
  • Highly developed communication skills enabling client and staff issues to be responded to in a logical and considered manner.
  • The ability to work with a team of staff promoting a positive commitment to a customer service culture and backfill Team Leader during periods of planned leave.
  • Commitment to safety and quality assurance for clients and staff.
  • Intermediate skills in Microsoft Office applications and previous experience with Aged and Disability Services software would be an advantage.
Mandatory pre-employment requirements:
  • Be willing to undergo and complete a satisfactory criminal history check
  • Provide or obtain an employment-based Working with Children check
  • Be fully vaccinated with at least two approved COVID-19 vaccines or have proof of medical exemption, in accordance with Council's COVID-19 Vaccination Policy
  • Current Victorian Drivers Licence
