There is now a long-term contract on offer for a dedicated, caring, and professional Social Support Officer to be the initial point of contact for new and existing clients for the many great offerings our Positive Ageing & Inclusion team provides with our in-home and community services.
As our new Social Support Officer, you will play a key role in supporting vulnerable people who are over 65 years and are socially isolated to build capacity, maintain independence and facilitate social connectedness.
Following the assessment of client needs you will actively support and develop volunteers to deliver strong community outcomes within the funding guidelines and aged care standards, while promoting a positive and professional image of the City of Marion.
Key components of this role include but are not limited to some of the following.
- Adhering to the Commonwealth Home Support Programme (CHSP) manual, Aged Care Quality Standards, and the City of Marion Values.
- Responding and actioning new client referrals, undertaking home visits and working with clients to develop care plans, achievable goals, and the implementation of services.
- Working in collaboration with new and existing clients to assess needs, tailor care plans and develop strategies that will facilitate their wellness goals and outcomes.
- Coordinating client booking requests and volunteer rosters.
- Supervising volunteers, including recruitment, induction, orientation, mentoring and sourcing appropriate training.
- Monitoring and reviewing client progress and volunteer development by setting strategies to achieve great outcomes.
- Developing and implementing new programs and initiatives that respond to community need and aged care reforms.
- Participating in the development and review of procedures, work instructions and streamlining operational processes to improve efficiency.
- Participating in the gathering and analysis of client feedback to establish the value, satisfaction and expectation for each service provided.
- Monitoring and reporting on key performance indicators related to Social Support Programs.
To be considered for this position you will need to have a good working knowledge of the Commonwealth Home Support Program (CHSP) Manual and Guidelines. A Certificate III in Community Services/Business Administration, or equivalent experience in a similar role is required with prior experience or an in-depth understanding of in-home services. A Diploma of Community Services and/or Certificate IV in Training & Assessment will be highly desirable.
A DHS Screening check is required for this position as well as a valid Driver’s licence and a willingness to drive.
