Community Therapy Service

• Join our collaborative and experienced social work team in Community Therapy Services
• Part time (4 days/week) or full time (with ADOs) based at the Royal Melbourne Hospital Royal Park Campus

• Provide exemplary social work service across but not limited to the Community Therapy Service, including psychosocial assessment, emotional support and counselling, and discharge planning.
• Demonstrate excellent communication strategies with patients, relatives, staff, and relevant services in order to provide appropriate services and information.
• Liaise with service providers and refer to relevant community services, Aged Care Assessment Service and Transition Care Program
• Enjoy the leadership and support of senior clinicians (grade 3) and clinical lead (grade 4) as well as the CTS multi-disciplinary team

• You have demonstrated health social work experience with a collaborative working style.
• You have excellent communication skills and a passion for continued professional development for yourself and others.
• You are a qualified Social Worker, eligible for membership with the Australian Association of Social Workers.
• Grade 2 applicants must have at least 4 years’ experience.
• You value working with supportive and dedicated health professionals and multidisciplinary teams.
