About the role
This role is based in the community rehabilitation services team, there may be requirement to move across the health service at times. You will be required to see patients in clinics and in their home environment
The Social Work team provides patients with additional support to improve psychological and social functioning and we ensure each patients social, emotional and cultural needs are met. We minimise the negative impacts of illness and hospitalisation on patients. We are a dynamic and innovative group of 30 plus social workers who are always willing to share knowledge and peer support.
There are regular department in-services, external professional development opportunities, you are able to access study leave for higher education. There are many opportunities to develop your career and knowledge
Key Responsibilities
Manage a caseload of general and neurological rehabilitation patients
Complete comprehensive psychosocial assessments and care planning
Working effectively in a multidisciplinary team
Counselling in context of adjustment to illness
Risk assessments in elder abuse/family violence
Linking with community supports
Skills and experience
Eligibility for practising membership of Australian Association Social Workers Membership (must have a bachelor or qualifying master’s degree)
Demonstrated ability to monitor, evaluate and modify own performance through reflective practice
Commitment to student education
Ability to adapt and learning quickly
Ability to plan and problem solve whilst working autonomously
Sound knowledge in Social Work Theory and Practice
