About the role:
Apex Health have an exciting opportunity for a telehealth Social Worker to join our team, assisting veterans to access Medicinal Cannabis for the treatment of their chronic health conditions.
Your role is crucial in guiding patients through the process of enrolling and participating in the program. You will serve as a point of contact for patients, providing them with the necessary information, support, and guidance.
You will work closely with our team of Medicinal Cannabis care professionals to ensure the utmost quality of healthcare for our veterans.
Responsibilities:
Conduct thorough assessments of patients' medical history, current health condition, and treatment goals. Use this information to determine their suitability for the program.
Assist patients in completing all required documentation, forms, and applications needed for enrolment in the Medicinal Cannabis program.
Maintain regular communication with patients, addressing their questions, concerns, and feedback. Serve as a reliable point of contact for ongoing support throughout their participation in the program.
Handle patient information with the utmost confidentiality and professionalism, adhering to all relevant privacy laws and regulations.
Coordinate with medical professionals, pharmacists, and other stakeholders involved in the patient's healthcare to ensure a holistic and integrated treatment approach.
Educate patients on responsible cannabis use, potential side effects, and strategies to monitor and manage their treatment progress. Encourage patients to actively participate in their own wellness journey.
Maintain accurate and organized records of patient interactions, assessments, recommendations, and outcomes. Use this data to track patient progress and contribute to program improvement.
Requirements for the role:
- Possess a relevant Social Work degree.
- Maintain current registration with AASW (Australian Association of Social Workers).
- Demonstrate a genuine interest in the field.
- Showcase proven high standard written and verbal communication skills.
- Display proficiency in computer skills and the ability to work with various software programs.
