This position will provide high-level clinical social work services, as a member of a community-based multidisciplinary team, within Gold Coast Hospital and Health Service, to adult clients accessing Chronic Disease and Post Acute Programs.
The Role
An opportunity has become available for a Social Worker – Senior (Chronic Disease and Post Acute Programs) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
- Demonstrate extensive clinical experience and expertise in Chronic Disease and Post-acute client care
- Identify and assess the complex bio-psychosocial and counselling needs of clients/consumers families and carers
- Demonstrate experience in the delivery of group education and therapy programs particularly Mindfulness/Acceptance and Commitment Therapy based programs or similar
- Apply high-level independent professional judgement, problem solving and decision making in managing a complex caseload
- Monitor service delivery and actively contribute to the development of Chronic Disease and Post Acute Programs and Social Work quality activities, research, clinical practices, procedures and protocols
About you
To be successful in this position you will require:
- Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
- Tuberculosis screening, to identify if the tuberculosis vaccination may be required
- Declaration of serious discipline history must be disclosed
- Criminal History Screening will be required for new employees (GCH will cover cost)

- Permanent part time
- Base salary ranging from $62.75 - $67.53 per hour + 12.75% super

The services are based at Robina and Helensvale, offering group-based education and exercise programs, and individual therapy sessions which can be delivered in centre or within the client's home
