About the role
The Senior Social Worker will provide leadership as a member of a multidisciplinary team to older people and their support network whilst on the Transition Care Program. There is a requirement to deliver high quality social work services through the provision of psychosocial assessment and evidence -based Social Work services to promote, maintain and improve the health, functional ability, and quality of life for older adults.
Conducting Aged Care Assessments as an Associate Assessor is a function of this role.
All WHS Social Work staff are required to participate in the Ipswich Hospital Social Work on call roster.
About you
Our ideal candidate will be someone who can demonstrate the following:
- Demonstrated high level expertise and skills in delivering assessment, intervention and independent decision-making reflecting evidence informed interventions to adults to ensure the maintenance of social work practice standards.
- Demonstrated high level of professional leadership using a solutions focussed approach applied to the current and potential clients of the program engaging cross discipline involvement where appropriate, whilst exercising a high degree of independence and professional decision making.
- Demonstrated effective written and oral communication skills in providing multidisciplinary care in a complex environment including psychosocial assessments, management of clinical documentation and reports to address client needs and high-level advocacy to ensure access to services.
- Providing a high level of clinical leadership in the supervision and development of staff including supporting effective and highly accountable social work practice and active ongoing professional development.
- Providing high quality Social Work advice to assist transdisciplinary care by effectively integrating the social work perspective and collaboration as required with government and non-government service providers to improve health outcomes.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
