Job Opportunity: Social Worker - Acute, Subacute & Outpatient

Location: Shepperton, VIC

We are currently seeking a dynamic and experienced Social Work Leader to join their team and contribute to their mission of excellence in healthcare.

Role Overview:

You will play a crucial role in delivering comprehensive psychosocial support to patients, families, and significant others facing illness, disability, or lifestyle changes. Working across inpatient and outpatient settings, you will collaborate with interdisciplinary teams to ensure the highest standards of care and continuous quality improvement.

Key Responsibilities:

  • Deliver a comprehensive, client-focused Social Work service across various healthcare settings including sub-acute, acute, and outpatient settings.
  • This includes providing support to patients in areas such as Rehabilitation, Geriatric Evaluation Management, Palliative Care, MCU@Home, Renal Services, Specialist Consulting, Ante Natal, and Oncology.
  • Engage in evidence-based clinical work and lead quality improvement initiatives.
  • Drive the implementation of best practices in Social Work interventions, ensuring outcomes are monitored and evaluated to enhance quality.
  • Collaborate within interdisciplinary teams and represent the Social Work Department in organisational meetings.
  • Work alongside healthcare professionals from different disciplines, respecting their expertise while contributing to patient-centered care discussions and decision-making processes.
  • Supervise and mentor staff, including clinical supervision of Grade One Employees and coordination of student placements.
  • Provide guidance and support to junior staff and students, ensuring their professional development aligns with organizational goals and standards.
  • Ensure compliance with healthcare standards, regulations, and organizational policies.
  • Uphold ethical and legal guidelines, maintaining accurate clinical records and participating in quality improvement projects and audits.
  • Provide exceptional service to internal and external stakeholders, maintaining professionalism and integrity at all times.
  • Collaborate with external health professionals and services, ensuring seamless patient care transitions and effective communication channels.

Key Selection Criteria (Mandatory):

  • Commitment to organizational values of compassion, respect, excellence, accountability, teamwork, and ethical behavior.
  • Approved tertiary qualification in Social Work and eligibility for membership with relevant professional associations (AASW).
  • Demonstrated understanding of person-centered care principles and ability to work effectively in teams.
  • Strong assessment and intervention skills for patients with diverse psychosocial circumstances.
  • Excellent communication, time management, and organizational skills.
  • Evidence of full immunization against COVID-19 and Influenza.
  • Satisfactory National Criminal History Check and Victorian 'Employee' Working with Children Check.

Desirable Criteria:

  • Knowledge of NDIS, VCAT, and OPA functions.
  • Previous experience working in a healthcare setting.
